The New York City Health and Hospitals Corporation (“NYC Health + Hospitals”, “we”, “us” or “ our”) is committed to protecting the privacy of all individuals who utilize the MyChart Patient Portal (the "Site"). Please read this privacy policy (this “Privacy Policy”) and our Notice of Privacy Practices to understand how we collect, use and safeguard the information you may provide to us via this Site.

ACCEPTANCE OF THIS PRIVACY POLICY
YOUR USE OF THE SITE IS GOVERNED BY THE TERMS AND CONDITIONS OF USE (THE “ TERMS”) AND THIS PRIVACY POLICY, AND BY USING THE SITE OR PROVIDING ANY PERSONALLY-IDENTIFIABLE INFORMATION, YOU ARE AGREEING TO THIS PRIVACY POLICY AND TO OUR COLLECTION AND USE OF SUCH INFORMATION CONSISTENT WITH THIS PRIVACY POLICY, OUR NOTICE OF PRIVACY PRACTICES, AND ANY CONSENT FORM(S) AND NOTICE(S) THAT HAVE BEEN PROVIDED TO YOU BY YOUR HELATH CARE PROVIDER OR BY US (COLLECTIVELY, THE “PRIVACY TERMS”), AND TO BE BOUND BY THE FOREGOING. HOW WE MAY MODIFY THIS PRIVACY POLICY
We may modify this Privacy Policy at any time. Any modifications made to will be effective immediately upon our posting the modified Privacy Policy on the Site. By accessing or using the Site, you agree to be subject to the Privacy Policy as posted on the Site at the time of your access or use. You agree to review the posted Privacy Policy each time you access or use the Site so that you are aware of any modifications made to this Privacy Policy.

WHAT INFORMATION DOES NYC HEALTH + HOSPITALS COLLECT FROM YOU?

Collection of Personally-Identifiable Information . The Site permits you to access certain of your information that you have provided to us, as well as to provide additional personally-identifiable information and health care information. By registering to use the Site and providing that information, you agree that we can use that information consistent with the Privacy Terms.

In order to facilitate Site navigation, improve functionality, and personalize and enhance your experience on the Site, we may also collect information through “cookies”, as well as by tracking certain technical information about your computer which may include elements such as location, operating system, browser, Site entry and exit points, and travel through the Site. We use cookies to help diagnose problems with our system and to administer the Site. We collect, retain, and use any personally identifiable information you provide to us for legitimate business purposes only, consistent with our Privacy Terms, including without limitation the following: to provide you information regarding your health; to provide our services; to operate or improve the Site; to monitor Site visitor traffic patterns and Site usage; to comply with applicable laws, regulations and rules, and requests of relevant law enforcement and/or other governmental agencies; to enforce the Terms; and/or to protect the rights, property, or safety of NYC Health + Hospitals, our users, and others.

Sharing of Information . We do not sell or rent your personally identifiable information to third parties. We share your information only with our affiliates, health care professionals and services providers, service providers for the Site, to systems and services that are used to provide the services or functionality of the Site, or as otherwise described in the Privacy Terms or in the Terms in order to provide you with the services available on the Site. In addition, we may disclose information we maintain, including personally identifiable information: (i) when required to do so by law, or may disclose such information in response to a request from a law enforcement agency or authority or any regulatory authority; (ii) to protect the integrity of the Site or the interests, rights, or property of NYC Health + Hospitals; or (iii) to enforce the Site’s Terms.

MINORS AND CHILDREN

We do not permit individuals under 18 years of age to use the Site, and we do not knowingly collect information from children. By using the Site, you represent that you are not under 18 years of age.

THIRD PARTY WEBSITES

The Site may reference or provide links to third party websites. We are not responsible for the third party websites, and you should review the privacy policies posted on such sites. Please be aware that we do not control, nor are we responsible for, the privacy policies or information practices of third parties or their websites.

PROTECTING YOUR PERSONALLY IDENTIFIABLE INFORMATION
Because security is important both to you and to us, we will always make reasonable efforts to ensure the security of our systems. Please be aware, however, that computers connected to the Internet and Internet data transmission cannot be fully secured, and that there is always a risk that security may be compromised and data exposed. We do not and cannot give you any warranty or guarantee that information stored on the Site or that you transmit utilizing the Internet is or will remain secure. You have the right to be notified when your health information has been acquired, accessed, used or disclosed in a manner that is not legally permitted, and where we determine that your health information has been compromised. You will generally be notified of a breach in writing, within 60 days of the event.
